Order Form Instructions
To request materials for PLATO Interactive Mathematics, please complete the following
form. There are seven steps, each asking different questions regarding your order.
Direct all inquiries to 800.701.0370.
Once your order request is received, you will be sent an e-mail confirmation.
If necessary, within one business day, you will then be e-mailed/faxed an Order
Authorization Form (OAF) for processing. Please sign and return all pages of the
OAF, along with a copy of your authorized Purchase Order. Once the order is processed,
you will receive an e-mail confirmation with the PLATO Learning order number for
your records.
